The 23-year-old broke a metatarsal in March and it has taken longer than the club expected for Kightly to recover.
McCarthy told BBC WM: “The first game of the season is probably a bit ambitious for him.”
He added: “He’s just finding it difficult to get back to full tilt. He can’t play in the Premier League until he’s fully fit.”
